Cat Adoption Application

If, for any reason, and at any time in the future, I am unable or unwilling to keep my pet, I agree to consult with Bridge to Bridge prior to rehoming or surrendering to any shelter. Bridge to Bridge will have first choice at repossession of my pet. I will work with my veterinarian and agree on a regular schedule for wellness visits, inoculations, internal worm preventative, flea/tick control, and any other tests we agree are necessary for the health and well-being of my pet. I understand that my pet has been screened and treated by a veterinarian. I acknowledge that because my pet was rescued from the streets, there may be underlying or ongoing issues and it is recommended I follow up with my own vet for a routine examination as soon as possible. I understand that once the adoption is official, I will be responsible for all financial obligations, and agree that Bridge to Bridge will not be held liable for any unforeseen issues. By submitting this application, I certify that the information I have given is true. I further authorize the investigation of any statement made in this application.
